Libmacgpg should be able to discern pinentry cancelled reasons

Currently Libmacgpg always reports error code 99 -
PinentryCancelled,
regardless of whether the user actually pressed cancel, or pinentry
itself
crashed.
If at all possible, Libmacgpg should be able to return different
error codes
for different reasons.
